Medical & Parental Consent Forms

In order to compete in any PSAL sport, every student athlete must turn in:

• A Medical Form completed once per year by a doctor
  **NOTE: please keep pages 1 & 2, ONLY TURN IN PAGE 3!**
• A Parental consent form which is completed for every sport an athlete competes in
